What is Hospice and Who is Appropriate?
Hospice is an approach to care that provides individuals facing a life-limiting illness and their family members with medical, spiritual and emotional support. Those entrusted in the care of our compassionate staff can receive hospice services in their home, in a nursing facility, an assisted living community or in our peaceful inpatient hospice facilities in Crown Point and Michigan City, Indiana. Hospice Franciscan Communities is funded by Medicare, Medicaid and private insurance carriers.
Hospice services are available to those with a life-limiting illness and who are no longer responding to curative treatment. Diagnosis may include cancer, congestive heart failure, renal disease, HIV disease, liver disease, Alzheimer’s disease, ALS and other general health declines.
The experienced Hospice Franciscan Communities staff works with individuals, their family members and their physicians to determine whether or not they may be appropriate for hospice services. Requests for hospice services may be initiated by a variety of people including physicians, social workers, nurses and family members.

Hospice Franciscan Communities recognizes that individuals receiving hospice services and their families may have many questions during this difficult time. Our nursing and social services staff works closely with each individual to ensure that all facets of hospice care are explained. In addition, our social services staff can explain additional services that may be available.
Providing Support During a Difficult Time
When an individual or their loved ones chooses Hospice Franciscan Communities, they have access to a broad range of services including education, bereavement groups, volunteers assistance and spiritual care support. Our diverse, experienced staff and volunteers stay in touch with family members for 12 months as they cope with the loss of a loved one.
